Why my sd card's data erases it self??

Whenever i reboot or turn off my phone my sd card data get deleted (not all of them) for example today i turned off my phone the music and videos got deleted.3 days ago i turned off my phone 3 apps on sd card got deleted.and also once i turned off my phone my photos got deleted .it is so annoying.i got a backup but it is so annoying please help me.

It sounds like your microSD card needs fixing its FAT to avoid mounted as EROFS (read only) when your phone is unable to write.
