Because I wanted my notification LED to work, I rooted the phone with Chainfire. Yesterday, the update to 5.0.1 went out OTA for my phone but while it downloads just fine, I get an error message shortly after it starts to install the update. Is it because the phone was unlocked and rooted? I did a factory reset and the update still won't install.

One of the things the update process does is check for root. If there are any signs that the phone is rooted, or has ever been rooted, it stops, because there are changes you can make while rooted that, if you update the ROM, will brick the phone.

Go into Settings on SuperSU and you'll see Full Unroot (at the bottom, in the Cleanup section). Try that. If you don't want a rooted phone. Because you probably won't be able to root 5.0.1 for a long time, if at all.
