Whenever I receive a call or make a call, screen goes black and can't turn it on.

Whenever I receive or make a call(so whenever I click to accept it, or dial in the number and click call) my phone's screen goes black and I can talk on the phone and all but I can't turn the screen on there for I can't even hangup or put it on speaker. Pretty much the only way to hang up is to hold the power button to reboot my phone. I have a Samsung galaxy s4 that is running a cyanogenmod 11 snapshot(4.4.4). How do I fix this? Should I use a different dialer app? Which ones are best?
