Welcome to Android Central! Probably a failing battery. Can you try a new or different one?

Considering the age of the phone, if you normally deep-discharge the battery (to around 5%), it's probably shot by now. Charge when it falls to 40% and it'll last for years. Condition a new battery first (see The Care and Feeding of Lithium Polymer Batteries).
