    Beginning several days ago, there appears to be a request on my Samsung Galaxy S4 phone from my gmail account, requesting access. I opened one and requested it be discontinued, but it persists. I have not allowed permission. Is this an attempt to hack my phone? Does this mean my gmail has been hacked?

    Normally, I wouldn't worry about this so much. The Anthem security breach has made me more wary.

    It could be that you tried to sign into a website using your gmail address. Look at what is requesting the access.
