battery charging and dischrging very fast.sometimes phone suddenly reboots.discharging is very fast

sir .. from the past week my battery us draining very fast . it also charges very fast . while discharging it drops like 80% to 60% within minutes .. sometimes it suddelny restarts while at 35% or 40 % . i dont have any extra apps. . plz help and tell whats the matter
