Galaxy S5 on Tmobile - baseband version FOG6. Problem with Direct dial icon on home screen

Galaxy S5 on Tmobile - got the update for 5.1.1. baseband version FOG6, since then my direct dial icons from home screen just went away
Called Tmobile, after over an hour with the tech support factory reset my phone and was able to re-create the direct dial on home screen.
3 days later, back to square 1.
Unable to create direct dial on home screen
Anyone else seeing this issue
Any help appreciated

Welcome to the forums. Are you using the stock launcher? Have you tried this with a different one like Nova?
