16GB sd card not recognized on my new phone, help?!

Yesterday, I bought a Samsung Galaxy Grand Prime Value Edition smartphone but my SD card doesn't seem to work. I had the same card on my previous smartphone (a Samsung Galaxy S) and it worked great. I did everything, I inserted the card multiple times and even tried to format it on my computer but nothing happened, the mount option is still unavailable because it says "Insert an SD card". My PC and my old phone recognize my card, so could this be an issue of my new phone? What should I do? Thanks in advance.

Welcome to Android Central! Do you know what filesystem the card was formatted in? FAT32, exFAT, NTFS? FAT32 is the most universally compatible.

Nothing happened when you tried to format it? (The card is shot.) Or it formatted but the phone won't recognize it? (The SD port in the phone could be bad.)
