Is the phone rear branded on the glass or the frame?

We wanted to stay with the Jump program but discovered just yesterday that since we cracked the back glass we can't with the phone in that condition. The device was purchased in early July and has only that issue. We plan to replace it but noticed there are two lines of information about the phone in tiny writing at the bottom. Is that writing, along with the Brand name and model printed on the frame of the phone or the glass? Basically, if we replace the glass, could we still be eligible for the Jump program?
