Verizon Moto x won't dial out

The last month my moto x will not dial out our receive calls. I can send a text and get online with no problem. It seems like the last update jacked my phone up. I tried switching between global and LTE/CDMA and it did not help. Also shows service state voice out of service data n service. Any suggestions?

I'd suggest taking it in to the Verizon store and getting a new SIM.
