SD Card isn't reading.

[AC Question]

Hello! Suddenly my SD card stopped working. If I get it out, then my phone shows that I disconnected it, also shows if I put it back. And when my card is plugged into my phone, and I connect my PC with my phone, then my PC can't detect it. When I connect my card it says some "Checkin for errors." stuff which never really goes away, only after some time, and it still dosen't work. Help me please!
