Why isn't my SD card showing up?

I recently ordered a PNY 128gb SD card for my galaxy s7 edge, and when I installed it, it was reading it fine. I followed the steps with adb to set 90% of it up for adoptable storage, which also went fine. When I unplugged my phone from the PC, the storage disappeared and upon a restart I got a notification saying my SD card needed to be reinstalled and that it wasn't being read. I've tried that a couple times, to no avail. What's going on?

NEVER remove an SD card set up as adoptable storage - it's part of the phone now. Reset it up and leave it in the phone. (It's a terrible idea, but Suhn won't release their technology that does it right.)
