App icons disappear from screen after restart

If i add apps to home screens or build folders of apps on homescreens, after i restart my phone, the apps(icons?) and folders are gone. I have tried different folder apps and different launcher apps with the same results. Do apps have to be stored on the phone instead of the storage card for startup to recognize them and place them on the homescreens?

No, they should stay on the screen. The phone has a problem, most likely with the firmware (which can be reflashed).

What model phone?

I have a ZTE Grand MAX X running 5.1

I had no issues with my old one running 4.4 (this one is a warranty replacement)

Also having issues with selections not being saved like "use this app - Just Once - Always" I choose always but continue to get prompted.
