Why does my Moto G 3rd gen stop recognizing my SanDisk 32gb microSD card?

I formatted the microSD card as internal storage and it has worked fine for a number of months. All of a sudden the phone stops recognizing the card. This isn't a case of not seeing the card at all (like I have seen in other posts on the web), so I don't believe the card is dead/corrupt. Here, the card is visible for a short time after powering on. I can navigate to photos & music files, and can run the few apps loaded to the card. Then after a couple of minutes I get a message saying SanDisk SD card missing (with the request to re-insert the device or Forget it). At this point the files and apps have "disappeared" and can't be accessed. This seems like a software problem, rather than hardware. I have rebooted the phone and cleared the cache partition. Phone is running Android 6.0 Marshmallow. It's frustrating to see all of the vacation photos for a short while, but not long enough to pull them off to a PC. Help is appreciated!
