earlier today when I unlocked my phone I noticed that 2 of the app icons changed. they are now white squares with green lines at the top and a little sd card and android icon in the square. what does this mean? why did only those 2 apps change? how do I fix it?

Is that only on the homescreen, or also in the Settings>Apps list? If it's only the homescreen, try removing the shortcut, then adding it back again.

Are you using an SD card?

I had the same thing happen 10/14/16. 3 of my app icons are now generic green android icons. The apps still work, but what happened to the icons? Nexus 7 (2013) tablet. No I don't have SD card.
