Howdo I send my Galaxy Express 3 while still under warranty

After forgetting my Pattern Lock. my Google account Reset, Erased and Locked My phone, and Sim's Card. I don't remember my Gmail account or Password Synced to my phone. my 2 yr.is still Effective. What address and how do I Send my phone to.
