Knox sensor tripped without rooting my phone? - Can't update OS

[AC Question]

I've had my phone since new, I went to try to update my OS (OTA) and it said i'm running modified software. I looked online and someone said rooted phones can't be updated. I know my phone isn't rooted, but I checked to see if my knox sensor was tripped. It shows "1" which means, it was tripped once.

How does this happen without rooting? and what can I do to correct this so I can receive the newest OS update?
