To do a factory reset go to settings>privacy>factory data reset. To format your SD card connect it to your computer (you might need an adapter if your computer doesn't have a micro SD slot), right click, and choose format.

Your instruction manual should tell you how to do a factory reset. Formatting you SD, if you wish to ship one with it, to not do so, would be very cheap, but standard FAT32 will be fine.

That is true on some phones.

On other phones a factory data reset is:
Home > Settings > SD & phone settings > Factory data reset.
