Using Apex Launcher (it's in the play store) to reorganize the layout and functionality of the screen.

The background is a photo he took - long click on the screen, select Wallpapers, Gallery, then whatever picture you want to use.

Weather and clock is from the Minimalistic Text widget collection - download from play store, long click on screen, Widgets, chose the widget you want, drag it to where you want it, then configure it to meet your needs.

Icons are from the Metrostation icon pack - download from play store, long click on any existing icon, Edit, click the picture, Icon Pack, MetroStation, chose whatever new icon you want, select a color, confirm, and it's done!

- Metrostation Icons (to replace the stock icons with plain white ones)
- Wallpaper is a photo I took (edited on the N4).

Lustre Icon Theme

i downloaded this, changed a few icons to other ones i liked from the same theme, and now my nexus is acting slow when going through screens.

edit* fixed itself after i rebooted.

Yes you would need to be rooted and have a flashed a different recovery such as TWRP or CWM. AOKP is a custom rom. There are guides here and other places that will help you through it, if you decide to. It's not a official build, its in Alpha stage but the build I am on is having no problems for me. Since your new to all of this I would suggest waiting until there is a more stable official build out.

I don't know how this happened somehow this happen when I hit the power button the lock screen still worked so didn't the widgets any one knows what happened here

Sure.. Mine I just used SimpleText to create the icons, and Nova Launcher which let's you customize the icons and rows/columns
