Default Flash Write Failure after softbrick

So I was using my nexus 4 today (unlocked bootloader but with factory image OS) and all of a sudden a bunch of apps started crashing (facebook, hangouts, messenger). I decided this meant I should restart the phone. The phone hung in boot animation for an hour before I gave up. I restarted and tried a few more times but always stuck in boot animation. I went to flash the google factory image and got the following output. I have a feeling this is a hardware failure. Poor timing on part of my phone since I was hoping to wait for the nexus 6 before upgrading....

Anyone have any experience with this error?

Did you get anywhere with this? Exactly the same issue. Nexus 7 (2013) shortly after upgrading to 5.1
