My first HOX issue! Help!

So today my volume rocker decided it wanted to control Media Volume only whether I'm in a media app or not. Yesterday it controlled Ringer Volume when on the home screen (or any non-media screen ) and controlled Media Volume when playing a YouTube video etc. like it's supposed to.
What happened and how do I fix it?
I've rebooted, it's like the phone thinks there's always media playing.

I've seen this too on a couple of occasion.

I had to go thru my apps and find and kill the media app that was running (or thought it was running). The next time, a simple reboot fixed it.

Still have no clue what happened, but it has not recurred.

I'm stumped! I've "stopped" everything and rebooted multiple times and the problem persists. Any other ideas? Do I need to do a reset of the phone? How do I do that?
