Default Just got a Virgin Mobile HTC EVO 4G and its already been rooted....

I just bought this phone and it has been rooted. Thats all I know. I dont know what else has been done to it so I want to just restore it back to factory settings and start with a fresh root and maybe more. Can I just do a factory restore? The phone says it has a OTA update (software update: 2.95.652.5 {55.58MB}) available but I dont know if I should install that or not either...? Ive done a little reading and Im not sure if it has S-off or not. All i know is that right now the phone is running Android 4.0.3 and HTC Sense Version 3.6,
    Software Number1.14.652.0 71ORD
    Kernel version 3.0.16-g4211684
    htc-kernel@ui8000-build-144 #1
    SMP PREEMPT
    Baseband version 1.09.00.0706
    Build number 1.14.652.0 CL372137 release-keys
    Browser ver. WebKit/534.30

Anyone know what I can do to get this phone back to its original state? Thanks in advance and let me know if you need more info.

I think a factory reset should take you back to factory settings, just do it and start from scratch

I did the factory data reset and everything went fine. then I downloaded the update and applied it and my phone went into Clockwork Mod Recovery and said:
    CWM Recovery v5.8.0.2
    E: Bad boot message
    "time"
    Finding update package...
E: Unknown volume for path [SDCSRD:do
    wnload/ota_shooter_ICS_35_s_Sprint_w................
    ...................1.zip
    E: Cant mount SDCARDownload/OTA_shooter... etc....zip
    installation aborted

Do I need to start the root process over again? can i just install zip from sd card thru CWM? Sould I wipe Cache partition??
Just dont wanna brick my phone...

also: when I turned the phone back on I still have root. or at least the Super User app is still there and will run.
