First, the obvious: There is none of the plastic covering that comes on new phones still on there?

Yeah, my first thought would be plastic covering...but it almost looks like it's fogging up from the inside. At least that's how my 'water-proof' watch looks now for a few days after I go for a swim.

So OP, you already peel off any plastic protector that (I think) came on the lens area?

If water got in but now can't get out, the fog would actually not go away. Because of the very distinctively outlined border at the bottom of the lens, I'd say this is definitely something that's coming from the inside of the phone. It doesn't look like a layer of the glass coming off, it looks like there's either water vapor or even an oily residue in there.

Yeah, this is supposed to be sealed, but phones fail sometimes and seals wear out over time, especially if handled roughly (and the inside of a pocket CAN get rough sometimes).
