Help please!

Anyone can help me with a strange issue?

When I just got my brand new LG Optimus S it showed two NAM (NAM1 and NAM2) profiles in QPST.
But after I rooted, changed ROM back and force, reset, restore, etc. I see only NAM1 available now (see attached pic). :'(

Question_1: How to return NAM2 back?
(I've already tried to restore back to Stock Sprint ROM, but this didn't help).

Question_2: Where is the information about NAMs stored?
