This can occur for a number of reasons, however I'm not sure why it would occur on Stock firmware (AOSP, CM, other ROM, etc. sure; stock, no idea). Most likely, any device affected has the install apk on the device, so all one should need to do is enable unknown sources under security settings...

Could the OTAs being received after flashing factory images directly from Google be an OTA that was pushed out by Google and not by the carrier?

Re: Root or not root The likelihood of any non-bootloader unlocked device attaining root on lollipop is highly improbable and more likely than not, will not occur. It's not impossible, but highly improbable due to the way Google has finally locked down it's OS to close serious security...

Touchwiz is awesome, but lacks some crucial features like customizing icon layout size. There's no reason 5.7' of screen needs to be filed up with 5 a 5x5 icon layout... unless you are extremely old or have extremely bad eyesight. Then again, that same complaint goes towards LCD density...
