Android Wear notification shows 'disconnected' when it's not

The Android Wear notification on my phone says "disconnected" but if I open the app itself (or the moto connect app) it says "connected". And it IS connected (full functionality on the watch, no cloud with bar through it etc). If I reboot the phone (Nexus 4) or the watch it will say "connected" at first but soon (within half an hour maybe) says "disconnected" again.
This is mostly just an annoyance. But is anyone else having this problem?

Happens all the time, just deal with it. Like with most non-hardware problems, it either goes away or you become used to it that it doesn't bother you anymore.

Update: although I haven't received the new update as of this morning (9-26) I noticed yesterday that my false 'disconnected' issue has *mostly* gone away. I don't think I changed anything or installed/removed any apps. I wonder if Google pushed a Google Services update to Android behind the scenes.

Update: after the KGW42R update the other day the issue seems to be fixed.
