Moto missing some calls on VZW?

I have the Moto X on VZW and I'm having a situation where I'm not getting some calls and notifications.
For example, my father called last night and I got the call, but a friend called twice and the phone never rang and there was no notification of a missed call.
Anyone else seeing this?
Would it be a device issue or a VZW issue?

What networks and phones are they on?

The friend whose calls never came through is on AT&T.
Not sure of her phone but it's pretty dated.
Probably a Froyo device.
Strange thing was...she called this morning and the phone rang and call was fine.
Very annoying.
