Droid 4 dying randomly, battery indicator not working, not always charging

My wife's phone dies randomly all the time. It will say it is charged up and then shut off. Sometimes the battery indicator shows a question mark, like its unsure what's going on either. Its almost like the phone doesn't think there is any battery, so it dies, even though there is a charge there.

Anyone else deal with this?

For the battery problem, try discharging your battery until it reaches the 10% alarm. Then shut the phone off and recharge it, not turning it back on until it's charged to 100%. This should reset the battery.
