Droid Turbo (motorola) receives repetitive pairing requests notification all the time

I tried to clear data and cache in Apps under Blue tooth share after I removed all bluetooth profiles but this did not work. I even went to verizon and got a new sim card. I also did a factory reset and still have problems. Please help

Is it just from one BT device or from any BT device?

I have two blue tooths and i am connected to both but the "phantom" keeps asking to pair. I don't know what device it is coming from?
