The Xposed Framework provides the structure to make modifications with out having to make APK changes or rom modifications. The Framework doesn't make any changes by itself. You have to load modules which provide the changes. You can install modules from the Xposed Installer or as APK installs. Since there are no changes made to the OS itself you can use OTA upgrades by disabling Xposed, upgrading and then reenabling.

A few of the modules I'm using are:

Advanced Power Menu: Customize your power menu to add reboot (normal, soft, recovery, & bootloader), screenshot, & more.
GravityBox - All-in-one complex module for devices running vanilla Android. Note there seperate modules for KitKat and JellyBean.
AppOpsXposed - Make AppOps accessible again in 4.4.2
