I just got my Neo hybrid on Saturday. For me, it's not that the monitor doesn't work, it's that the case is too bulky to allow my finger to rest on the sensor.

Yes, bad design, I agree. It's going back.

I have both the Neo Hybrid and Ringke Slim... I like the Ringke case much better... Although Spigen provides better overall fall protection, it has few issues: 1) Heart Rate Monitor never worked once for me whereas with the Ringke Slim case, it worked every time 2) Only certain microUSB chargers worked with Spigen case... if you have any OEM charger that has a bigger head, it will not work... not an issue with Ringke 3) Also, sometimes... the polycarbonate frame opening near the charging port can be sharper and inconvenient, especially during one-handed operation... I have filed a claim to get my money back with the Neo Hybrid and am planning to use only the Ringke case for a while unless I see a better case that I like.
