Samsung Galaxy Note 4 sd question

I'm new to the android world and was wondering if I have a bad sd card or what.
On my note 4 I transferred some of my apps to the SD card and when I restarted my phone all my pictures and videos were gone and the apps that were saved to the SD card are now grey. When I click on the app It says not installed. Also it does not recognize my sd card anymore. So I remove the sd card and re-install it and it says it's blank and needs to be reformatted.
I'm so confused. Please help. Is my sd card corrupt

Sounds like it may be corrupt. What you can do is connect your SD card to a computer, copy all the contents from your SD card to the computer, stick the SD card back into the phone, format the card per the phone's instruction, then connect your phone to your computer and copy the contents back over.

One of the biggest things I've noticed with some users is that they will throw the SD card into the device and won't bother formatting it. The best way to handle a new SD card is to throw it into the device it will be used in, and use that device to format per the file structure that best works for the device.
