Welcome to Android Central! "mmc_read failed" suggests a problem with the onboard memory. You may have to bring it to a repair shop.

Hi there. I get this issue as well as the unplugging issue. It's a massive headache for me. I have wiped the phone and used insurance to get a new one on Monday, a replacement Note4. Will post back if I see any differences. Perhaps the ROM is knackard.

Same issue. I had Anker replaced my batteries, and I sent the phone to Samsung for repair. Using my S4 now until I get it back.
