No, it's the phone.
My Captivate (Galaxy S) did not have this problem at all. Drove an Altima. No noise whatsoever. Got a GS3, and the noise was immediately apparent. I first thought it was a dirty aux cable, but when I used the Captivate's cable with the GS3, the sound remained.
So it's not the cable.
And I got a new car, Chevy Cruze, and the sound is even worse.
Someone recommended a ground loop isolator. I just got mine today and will check it out on my commute in the morning. If it works, I'll report back with a link.

The ground loop isolator works beautifully. Plugged the phone into the charger and aux port and immediately had noise. Plugged the ground loop isolator between the aux cable and aux port, and all noise was gone.
Not a single crackle or bit of hiss.

I have the same problems, I called Samsung Tech. He told me to do hard reset. Still the same. Then I thought maybe its the STOCK MUSIC PLAYER.
So I tried many music players. I found the 1 that works." Poweramp"..No more crackling, hissing,popping sound, nice crisp clear sound..SOLUTION= DL and installed Poweramp and .use Poweramp .instead of stock music player. I have the same problems. I called samsung tech. He told me to do a hard reset. still the same. than i thought to myself maybe its the stock music player. So i tried many music apps. I found the 1 that works. No more crackling,hissing,popping noise. its an app called" Poweamp".
Solution DL and use Poweramp instead of stock music player. I know the Poweramp apps cost $4.99 but its worth it.

also, lifehacker had a piece on using that audio garbage as a way of finding your phone (in your house or office) by remotely turning on your stock music app and following the static-y noise. My problem: how do you turn on the phone remotely?

If I turn the stereo volume waaaaay up (mine goes to 40, so anything over 34 or so), I can hear some of the hiss. But at normal listening volumes, it filters out all perceptible noise. I've been using it for a week now, and I haven't noticed any hiss or crackles at all (except for the one time I jacked an Edguy song to 40 and heard some of the hiss).

Hey guys,
I’ve just bought myself a white GSIII and has recently noticed that if I play sounds from any source (YouTube, Videos Player, Music Player, ringtone) etc, on near maximum volume, the sound on the loudspeaker becomes a bit distorted/ muffled. It plays fine on low volumes. I wonder if it was the way the metal plate grill on the loudspeaker was designed; how it has a lot of small holes on it and perhaps these limit the amount of noise coming out from it.

On my s3, when plugged into either headphones or the aux cable in my car, there was an intermittent clicking noise that would occur when listening to any audio from any application. I returned that one, and the new one had no issues.

Funny, I just got my GS3 on Saturday and I used the stock headphones with no issue. On my way to work this morning I plug the phone into my car Aux port and thats when I started to hear it poping and crackling. So as soon as I got into work I tried it with a pair of BOSE headphones and still the same result as if I were in the car, but hear is the strange thing after the first hour of playing music the poping and crackling is almost gone. I mean I can still hear it a little but not to the same effect as this morning in the car. What do you guys think?

Just got an AT&T Samsung Galaxy S3 and same issues reported here. I get noise popping and cracking while charging and using aux port. If I unplug the charger and noise goes away. I've tried multiple chargers with no fix.
Maybe I should go back to iPhone? I've used iPhones up until this point but decided to take the leap as jailbreaking is becoming more and more of a nuisance. This might just be a deal breaker though. Its a glaring oversight on Samsung's part. How can you compete with iDevices if you can't charge and play at the same time?
I shouldn't have to buy ground loop isolator either. That's a band-aid not a real solution.
edit to add: Buying a special music player app isn't a solution either. I like to use multiple apps, Pandora, Tune-In Radio, NPR, etc not just listen to MP3's. Noise comes through ALL of these apps on my phone. - 10-02-2012, 02:45 PM #72

I took mine back and got a new one. It works perfectly, No issues! Word of advice try listening to the music before walking out of the store to ensure that you did not get one of the bad ones.

The first post in this series indicates that the author tried an store display model with the same problem. I just got my S3 and I must say, I put in a lot of time researching it. The biggest problem I found in reviews was complaints about the display brightness. Obviously this is not the worst of it. I'm returning my S3. It's a shame since the S3 was on top in every comparison I read.

So I have been battling with this issue since I got my Verizon GS3 and I seem to have finally fixed it. Just flashed Jellybam rom last night and voila crackling over blue tooth completely gone. When I was stock and also on clean rom the crackling was horrendous. It must have something to do with the the touchwiz blue tooth because with aokp everything is fine. This rom is also awesome. Finally 100% happy with my S3!:-)
