Icons are blackbearblanc carbons and text. Folder icons I got from the play store awhile ago part of a theme called go apex I believe. Time weather widget is zooper widget. Flashlight with widgetsoid and screen off button from play store.

That would only work on the galaxy s3 if I'm not mistaken. A Google search brought up pressing and holding volume down and power at the same time for a brief second as the method for the galaxy nexus. According to his signature that's what he's using.
