Twice this has happened to me in the last 4 days. I was drinking the other night and figured out how to get it back, now I'm not drinking, and I can't remember what I did! I can't figure out why it's disappearing, I'm am 99% sure I am not deleting the shortcut by accident. I am running stock, with Nova Launcher. When I switch back to TouchWiz, the icon is there. Should I just uninstall then reinstall Nova? I remember doing some searching on Saturday night, and found very little on the issue, so I do t think it's a common problem. No, I don't intend on having a drink or two to help myself out, had enough this weekend :-)

Figured it out. Long press the home screen, select Nova actions, select App Drawer. Pin to Home Screen. That took me well over an hour on Saturday night, only 5 minutes this time. We'll it's documented for next time, just wonder why it did that to me twice now?
