Call volume keeps changing!!

Can somebody please help me. The call volume on my S4 keeps changing to default middle volume every time I make a call.
When I make the call I have to manually press the toggle/side button to ensure the call-ear volume goes to maximum, how to I permanently set it so it stay at max??

I believe the poster said in call volume. I have a Note 2 with the same problem, only the ear piece volume goes to high with every call, no matter what I do. I even downloaded Volume Locker, but it does not lock the volume. If I figure it out, I'll post it here.
