Why does my phone say it is charging but it isn't?

My phone says it's charging but isn't...

I have a Galaxy S4. My phone says it's charging with the little lightning bolt but isn't, the percentage is going down. I have tried restarting, taking the battery out, and using a different USB cable. Nothing is working. Please help.

Welcome to Android Central! Are you plugged into a wall outlet or a computer USB port? If you're using a wall outlet, does the phone say "Charging (AC)" or "Charging (USB)" in the Settings>Battery menu?

Also, are you using any battery-intensive apps while charging? If so, that might be outstripping the charging rate.
