Icons disappeared from home page

I was using my phone earlier, everything was fine.
I looked at it later, all my folders and icons are gone from my home page.
The only thing on the page are the four things on the bottom. Phone, Chrome, Contacts and apps.
I can get to everything from the app drawer, should be a simple matter to repopulate my home page.
My question is: Why did this happen?
I've restarted my phone several times.
I've both just restarted, and shut down, then restarted.
All of my history seems to still be in the apps, I.e my email accts are signed in, my s-health history is there.
The only thing that's changed are the icons have vanished from my home page.
Phone is bone stock

Are you using stock launcher (Touch Wiz) or a third party (Nova, Apex, or etc)?

When you were on the phone, it could be possible the proximity sensor didn't catch that the phone was near you, and you accidently deleted your home page. (Pinching screen, then dragging page to the trash can.)

You may want to consider installing the Nova Prime launcher. You can lock your home screens so the icons don't disappear or move.

I guess you missed my last sentence that said "my phone is bone stock"
I.e, using touchwiz

Having said that, your explanation is definitely plausible.
Not a big deal though, since all it did was delete my icons. Took me all of 5 minutes to get my screen back the way I had it.

This happened to me the first week i got it. Never has happened since.
