Sound volume inconsistent

Has anyone encountered issues with sound and volume? When using headphones, the key touch sound is way too loud, but very quiet without headphones (ext speaker). A few times the music volume has been fairly muted until I restart app. With all volume levels turned low, ringtone selection menu blasts a sample of selected ringtone. And sometimes ringtone sounds distorted, like when you over drive an amplifier. I can't say I've done thorough testing on anything, but wondered what others have experienced and if you found solutions. My guess right now is that it's software related because when speaker sounds good, it sounds really good.
