Media library will not update
My last phone was the S1, and every time I disconnected the USB cable from my computer the library would rescan. I was thrilled that the S3 didn't appear to do this (or maybe it's just so fast that I don't notice anymore). I could throw on a few albums and they would instantly appear in the Music Player, regardless if they were in my /music folder on the 16GB SD card or the phone's internal storage. Awesome.
However, ever since the OTA update I got a couple weeks ago, I can no longer get new audio files loaded into the media library. I've rebooted the phone. I've cleared the cache for the Music Player app. I've rescanned the "Music Squares" thingy. I've tried adding a variety of different albums on the SD and internal storage alike. I found an app called sdrescan (which is supposed to rebuild the library), but it just won't update. The only change that sticks is if I delete an album -- the Music Player app picks up on this.
I know the files are there. "My Files" shows them. I can even tap on them in that app and manually launch a single mp3 from the default "Sound Player" app and they play fine. The files are not corrupt. There aren't any weird characters or excessively long folder/file names.
Any ideas on how to get this to kick in?

Well, I think I was able to figure it out for the time being.
I tried unmounting and then mounting my SD card but that just removed every song from my library that was stored on the card. No way to add it back in -- reboots, clearing Music Player cache, etc. The only way to get the songs recognized was to backup the SD card, format it, and copy everything back in. Maybe there was a problem with the SD card's file system that developed over time?
This also did not fix my inability to add songs to the phone's internal storage. I had to delete the entire /music folder and re-add it. This got everything to kick back in.
Doesn't make much sense, but I'm just happy it's working again. Hopefully this is a one-time thing.
