Can't Factory Unlock after root

I rooted my phone a little over a year ago. Due to other factors, I factory reset it, and it upgraded to current stock for SGH-T999. Contract is up and they sent me the factory unlock code, but it will not work.
I took it to a phone repair shop and they could not unlock it either.

What to do?

Hold on, SGH-T999? T999 is S3 if I recall. It upgraded automatically via OTA? What steps are you following to unlock it?

I had the same issue on my rooted S2 skyrocket. I could not unlock running CM11.

Reflashed to stock, and the unlock code worked. FWIW, I unlocked through the automatic prompt it gives on startup using another carrier's SIM.

Duh, You're right..... my bad, wrong forum. My new phone is an S4, and my old is an S3.
