4G won't turn off

Help, had to get a replacement phone and I noticed that my 4g now won't turn off. What can I do to fix this.

You mean that if you turn off mobile data, the 4G icon remains in the Notification Bar?

No, sorry just woke up. Lol when I turn wifi on the icon for 4g remains there.

It's a known issue in the 4.1.2 Verizon build. Go to Settings -> Application manager -> ALL, scroll down until you find 'Caller Name ID', select it and Disable it - problem solved.
