How to Prevent the mysterious mis-locating apps on my phone?

Once a while, my android ice-cream sandwich always do weird stuff. Like my app will randomly disappeard from one place and being located on other side of the page. Sometimes, changing batteries also makes the mess of my apps location on my phone.

Anyone has similar experiences too? Is my phone or system got bugs?:-[

My Atrix used to do this. I would fix if by choosing "sort apps installed earliest first" and then go to "sort apps alphabetically" and that would put them back where they were. I don't know why it did that. That was my work around though.
