Screen Lock set to PIN, but it it intermittently changes to "none"

Recently, my Droid Maxx has stopped requiring a PIN to sign in, and it does not automatically lock at the time interval at which I set it. The screen goes to sleep and shows a lock icon, but the screen unlocks by simply swiping the lock icon downward, even though the phone is set to unlock using a PIN number. Never seen this happen before. When I check Security and Screen Lock, it is still set to "PIN," yet the phone is acting as if it was set to "none." The only things that seem to cause it to go back to requiring a PIN to unlock the screen is (1) set it up to require a PIN (again), or reset the phone. At that point, it starts requiring a PIN to unlock the screen. But sure enough, after a period of time, it stops requiring a PIN to unlock the screen. Any solutions?

That sounds like a Timeout issue. Sometimes a screen can not lock right away. I would see if there is anything about locking after a certain time within the security area.
