    Which album/folder does it show up in? Might be that an app you installed downloaded this picture for any reason (e.g. might be a picture used in a game, accidentally downloaded from social media, etc.).

    Welcome to Android Central! I agree with mr_master -- it's common for an app or an ad network associated with an app to cache images in their own directory. If the developer wasn't smart enough to include a file called ".nomedia" in that directory, then those images will show up in the phone's Gallery app. (.nomedia instructs the system to ignore that directory when scanning for images to include in the gallery database.)

    Using your gallery app, select that photo and tap the Details/Info option. What directory is the image in?
