Phone doesnt ring; call does not show in Call History

Ok, now 2014...HTC REZOUND thru Verizon. I still experience two separate but related issues: 1) phone does not ring on incoming calls & goes to VM; only way I know is if VM was left. 2) Call does not show up anywhere in Call History. Happens periodically. Number not blocked, phone not in use at time of incoming call, batt not low, or any other Settings explanations I have seen in A. Forum back to 2012. Help pls!
