When you bought your Note 4. Did it come with a little plastic on the back of the heart rate sensor?

Mine did not. There was, however, a plastic film with a hole in the center which covered the camera lens. Mine is an at&t Note 4.

Did you buy it untouched and not set up, from a distributor? Or did you buy it from a carrier or big box store? If the latter, they usually remove all the protective plastic before giving you the phone. (Although one of my Note 3s still had a thin strip around the edge that thy missed.)
