Why am I getting the popup Conditional Call Forward Active when I try to make a call?

Conditional Call Forward Active

Whenever i try to make a call on my samsung galaxy s5 a message appears saying "conditional call forward active" and then cuts the call off. I am not able to send texts either. It has been like this for 3 days now, i cannot think what could have caused it.

Does anyone have any suggestions of what could fix this?

Welcome to Android Central! Can you turn off Call Forwarding? How to use call forwarding on my Samsung Galaxy S5

It might also be something that is set in your wireless account, and not the phone. Call your carrier and see if they can help.
