"Note to Self" no longer works!
All of a sudden, my primary HTC One has forgotten who I am! Normally, "Note to Self" works properly. Now it's asking me to enter a recipient email address. My other
HTC One works perfectly, as does my Galaxy Tab-4. Hopefully, it's a setting of some sort. Anyone have any thoughts?

It could be a speech recognition issue on the device. Go into Google Now settings > voice. Retrain the Google Now command and see if that helps.
Also, make sure you are clearly giving the command so that it doesn't mistake the command for sending an email to another person.

It recognizes "note to self". I also recognizes almost 99% of what I say. It creates (and saves) the note to self, but then asks for at least one recipient.

I have the same issue. I'm so tired of Android / Google stuff breaking. It seems like it works until they update something and then it breaks.
